To discuss treatment approach and therapeutic management of CKD in Primary Care in people with and without Type 2 Diabetes

To discuss person centered risk stratified preventive and therapeutic approaches to cardio reno metabolic comorbid patients in Primary Care
DAY 3: Digitalization in health care
To demonstrate the use of electronic health records for pro-active preventive care outreach into the community.
To outline possibilities for patient empowerment by using smart IT solutions
To highlight the importance of personalizing diabetes care with smart digital tools
To discuss the possibilities to implement CGM in daily PC practice for PwD
